耳前淋巴结肿大、肝脾肿大、腮腺肿大、泪腺发炎、眼睑浮肿、睑缘呈横“S”形下垂、葡萄膜炎、泪腺肿大、眼睑脓肿、腮腺肿胀

The pathology of disease

Chronic dacryoadenitis is persistent lacrimal-gland enlargement/inflammation, often bilateral, due to autoimmune/granulomatous disease (Sjogren syndrome, sarcoidosis, thyroid-eye disease) or lymphoma. Clinical: painless upper-outer lid swelling/dry eye over months. Treatment: investigate systemic cause (Sjogren/sarcoid/thyroid); systemic steroids/immunosuppression for autoimmune; biopsy to exclude lymphoma if persistent/unilateral.

Common Symptoms

Painless upper-outer lid enlargement over months; dry eye; bilateral; systemic features by cause

Causes and Pathogenesis

Autoimmune/granulomatous infiltration (Sjogren/sarcoid/thyroid) or lymphoma enlarges the lacrimal gland.

Treatment

Investigate systemic cause; steroids/immunosuppression; biopsy to exclude lymphoma

Preventive Measures

Systemic workup; treat autoimmune disease.
