Orbital inflammatory pseudotumor (idiopathic orbital inflammatory syndrome) is a nonspecific inflammatory orbital mass without identifiable cause, involving orbital fat/extraocular muscles/sclera/lacrimal gland. Clinical: acute painful proptosis, diplopia, eyelid swelling, and reduced eye movement; more common in adults. Treatment: systemic corticosteroids (usually dramatic response); immunosuppression/radiotherapy for recurrent/steroid-dependent cases; biopsy if atypical to exclude lymphoma.
