Macular Hole

Macular hole is a full-thickness defect at the fovea, usually idiopathic in elderly women due to vitreofoveal traction, or traumatic/myopic. Clinical: central blurred vision, metamorphopsia, and a round red foveal defect on OCT. Treatment: pars-plana vitrectomy with internal limiting-membrane peel and long-acting gas tamponade, with face-down positioning; high closure rate.

Causes and Pathogenesis

Vitreofoveal traction separates the fovea, forming a full-thickness hole.

Treatment

Vitrectomy with ILM peel, gas tamponade, face-down positioning

Early surgery; monitor fellow eye.
