Retrobulbar neuritis is optic-nerve inflammation posterior to the globe, so the optic disc appears normal on fundus exam despite acute visual loss. It is a form of optic neuritis, often MS-associated. Clinical: acute painful monocular vision loss with eye-movement pain, afferent pupillary defect, and normal disc. Treatment: MRI for MS risk; IV methylprednisolone to speed recovery; MS-modifying therapy if indicated.
