Dopa-responsive dystonia (Segawa disease, autosomal dominant GTP cyclohydrolase I deficiency) is a childhood-onset dystonia with diurnal fluctuation, progressing to parkinsonism. It classically presents as childhood dystonic gait that worsens as the day advances and shows a dramatic, sustained response to low-dose levodopa. Diagnosis is clinical plus genetic/CSF testing. Treatment: low-dose levodopa gives excellent long-term response, often preventing disability.
Childhood dystonic gait; diurnal fluctuation (worse evening); parkinsonism; dramatic low-dose levodopa response
GTP cyclohydrolase I (TH) deficiency causes striatal dopamine synthesis defect, responsive to low-dose levodopa.
Low-dose levodopa (excellent response); long-term treatment; genetic counseling
