Elastosis Perforans Serpiginosa

Elastosis perforans serpiginosa is annular/serpiginous keratotic papules on neck/face/arms in adolescents, associated with Down syndrome/Ehlers-Danlos/Marfan/osteogenesis imperfecta/penicillamine. Treatment: reassurance; topical retinoids/keratolytics; cryotherapy/electrodessication/laser; investigate associated genetic/connective-tissue disease; follow-up dermatology; distinguish from Kyrle disease/reactive perforating collagenosis.

Causes and Pathogenesis

Abnormal elastic fibers extrude through epidermis (transepidermal elimination).

Treatment

Reassurance; topical retinoids/keratolytics; cryotherapy/laser; investigate associated disease

Treat associated disease; avoid penicillamine if possible.
