Human Papillomavirus Infection

Papovaviridae = papilloma/polyoma/simian vacuolating virus; two genera (Papillomavirus, Polyomavirus), oncogenic DNA viruses.

Common Symptoms

Papulomacular rash; punctate bleeding; hoarseness; nodule; airway obstruction; squamous cell carcinoma; keratitis; asphyxia; condyloma

Causes and Pathogenesis

HPV spread by contact/fomites; genital = sexual; perinatal. Local infection, no viremia; types cause different papillomas. Skin warts benign (free DNA, self-resolve); high-risk HPV integrates into host chromosomes → cervical cancer. Cell-mediated immunity important; immunosuppression increases warts/cancer. Direct/indirect contact → microerosions → nuclear replication; local spread; plasmid/latent state.

Treatment

Drug, surgical, physical, supportive therapy

Preventive Measures

Avoid lesions; disinfect tubs/towels/toilets; diagnose/treat genital warts; condoms; sex education; multivalent/E6/E7/peptide vaccines in development.
