Mesenteric Tumor

肿瘤内科

Common symptoms

急性肠扭转、肠穿孔、便血、腹痛、静脉曲张、乏力、低热、肠套叠、肠系膜囊肿、腹膜炎

The pathology of disease

Primary mesenteric tumor is a rare disease arising in mesenteric tissue; limited Chinese data show it is more common in males and may occur at any age. Onset is insidious, with non-specific early signs; clinical misdiagnosis rate is high, about 2/3 of patients being misdiagnosed as other diseases or suspected as indeterminate abdominal masses.

Common Symptoms

Acute volvulus; intestinal perforation; hematochezia; abdominal pain; varices; fatigue; low-grade fever; intussusception; mesenteric cyst; peritonitis

Causes and Pathogenesis

(A) Etiology: Primary mesenteric tumor is rare and may arise from any mesenteric cell component; seven possible origins: lymphoid, fibrous, adipose, neural, smooth-muscle, vascular and embryonic remnants. (B) Pathogenesis: Mesenteric tumors are benign or malignant; benign:malignant ratio about 2:1. Malignant tumors are most often fibrosarcoma and leiomyosarcoma, mostly in the small-intestine mesentery and around it, also in sigmoid/colonic mesentery. Cysts arise from congenital anomalies (enterogenous, serous, dermoid), neoplasms (cystic lymphangioma), parasitic, traumatic (hemorrhagic), inflammatory cysts. Enterogenous cysts are lined by intestinal mucosa and other gut-wall layers, most often in ileal mesentery. Serous cysts are endothelial-lined, mostly in transverse/sigmoid mesentery; size ranges from centimeters to 20 cm, usually single unilocular cysts; fluid yellow-white/straw-colored transparent, or dark red/purulent if infected; rupture can cause peritonitis. Dermoid cysts contain semi-formed gelatinous material and hair. Cystic lymphangioma consists of dilated lymphatic vessels forming milky cystic structures 1-10+ cm, mostly in ileal mesentery, sometimes diffusely filling the small-bowel mesentery, likely from collecting-lymphatic obstruction, with clear or chylous fluid. Benign tumors include neurofibroma, fibroma, lipoma, leiomyoma, hemangioma. Solid mesenteric tumors are mostly malignant; lymphosarcoma has highest incidence, then leiomyosarcoma, liposarcoma, fibrosarcoma, mesenchymosarcoma. Liposarcoma looks lipomatous/myxoid/fish-fleshy; microscopically four types: well-differentiated, myxoid, round-cell, pleomorphic; first two have good prognosis; mitotic count predicts differentiation and metastasis (1 mitosis/HPF = 11% metastasis; >10 = 33%).
