Pulmonary edema is accumulation of fluid in alveoli/interstitium, either cardiogenic (heart failure/ischemia/valve disease) or noncardiogenic (ARDS/altitude/toxic neurogenic). Clinical: dyspnea, orthopnea, pink frothy sputum, crackles, hypoxemia, bilateral infiltrates. Treatment: oxygen; upright position; diuretics if cardiogenic; nitrates/afterload reduction; treat ischemia/arrhythmia; NIV/intubation if severe; treat noncardiogenic (ARDS ventilation/steroids for altitude/neurogenic); monitor.
Dyspnea/orthopnea; pink frothy sputum; crackles; hypoxemia; by cardiogenic/noncardiogenic
Elevated pulmonary-capillary pressure (cardiogenic) or increased permeability (noncardiogenic) causes alveolar fluid.
Oxygen; diuretics/nitrates if cardiogenic; NIV/intubation if severe; treat cause
Treat heart failure; avoid altitude exertion; manage fluid balance.
It is recommended to visit the Cardiology/Pulmonology-Emergency/ICU department; an appointment can be booked on this platform before the visit.
