Pulmonary Cryptococcosis

Pulmonary cryptococcosis is subacute/chronic visceral mycosis by Cryptococcus neoformans; saprophytic yeast, no hyphae/spores, ubiquitous. Immunocompromise predisposes; inhalation → pulmonary focus; mainly lung/CNS; also bone/skin/mucosa. Healthy often self-limited.

Common Symptoms

Station doctor; capsular polysaccharide accumulation; mucopurulent sputum; dyspnea; low fever; fever with cough; hemoptysis

Causes and Pathogenesis

Immunocompromise predisposes; healthy inhaled infection usually pulmonary-limited/self-resolving. Fatigue/immunodeficiency (late malignancy/leukemia/high-dose steroids/broad antibiotics/anticancer drugs) → pulmonary focus → hematogenous spread, often CNS. Respiratory route; lung = portal.

Treatment

Drug therapy, supportive therapy

Preventive Measures

Strict indications for antibiotics/steroids (dysbiosis/immunosuppression); consider cryptococcosis if no improvement; hygiene; no rotten fruit; prevent pigeon/bat feces contamination; boost immunity; avoid wound/soil/bird dropping contact.
