Chronic Cor Pulmonale

Chronic cor pulmonale is right-ventricular hypertrophy and failure due to pulmonary hypertension from chronic lung disease (COPD/interstitial lung disease), chronic thromboembolic disease, or hypoventilation. Clinical: dyspnea, right-heart failure (edema/JVP elevation), and signs of underlying lung disease. Treatment: treat the underlying lung disease; long-term oxygen therapy; diuretics for right-heart failure; PAH-specific therapy in selected cases; ventilatory support for hypoventilation.

Causes and Pathogenesis

Chronic lung disease/hypoxia raises pulmonary vascular resistance, causing RV pressure overload/failure.

Treatment

Treat lung disease; long-term oxygen; diuretics for RVF; PAH therapy in selected; ventilatory support

Preventive Measures

Treat COPD/ILD; smoking cessation; long-term oxygen.
