休克、传导阻滞、去首高血压、钙离子内流、室颤、猝死、心肌冬眠现象、低血压

The pathology of disease

Drug-induced cardiomyopathy is myocardial dysfunction caused by cardiotoxic agents, most commonly anthracycline chemotherapy (doxorubicin), also trastuzumab, alcohol, cocaine, and stimulants. It may be dilated (anthracycline) or non-ischemic. Treatment: discontinue the offending agent; guideline-directed medical therapy for HFrEF; monitor LVEF; dexrazoxane for anthracycline cardioprotection; early detection with echo.

Common Symptoms

Heart failure; reduced EF; history of anthracycline/trastuzumab; by dilated/nonischemic

Causes and Pathogenesis

Chemotherapy/other drugs cause direct myocardial toxicity and dysfunction.

Treatment

Discontinue drug; GDMT for HFrEF; dexrazoxane; monitor EF

Preventive Measures

Baseline/serial echo; limit cumulative anthracycline dose; cardioprotection.
