Left bundle-branch block (LBBB) is block of the left bundle branch, causing wide QRS with typical morphology. New LBBB may indicate acute MI. Chronic LBBB is associated with structural heart disease; in HFrEF with wide QRS, cardiac resynchronization therapy (CRT) improves outcomes. Treatment: treat underlying cause; acute - rule out MI; chronic - investigate structural disease; CRT if HFrEF with LBBB/QRS >130ms.
