Sick Sinus Syndrome

Sick sinus syndrome is dysfunction of the sinus node, presenting with sinus bradycardia, sinus arrest, SA block, or tachycardia-bradycardia syndrome. Clinical: dizziness, syncope (Stokes-Adams), palpitations. Treatment: treat reversible cause; permanent pacemaker for symptomatic bradycardia/syncope; for tachy-brady syndrome, pacemaker before AV-nodal blockers/antiarrhythmics; anticoagulate if AF develops.

Causes and Pathogenesis

Degenerative/ischemic/fibrotic disease impairs sinus-node function.

Treatment

Treat reversible cause; pacemaker for symptomatic bradycardia; pacemaker before AV blockers in tachy-brady; anticoagulate if AF

Preventive Measures

Avoid excessive AV-nodal blockers; pacemaker prevents syncope.
