Clonorchiasis

Clonorchiasis (Chinese liver fluke) = Clonorchis sinensis in intrahepatic bile ducts; raw/undercooked freshwater fish/shrimp. Mild asymptomatic; severe: dyspepsia/epigastric pain/diarrhea/hepatomegaly; cholangitis/cholelithiasis/cirrhosis. 24 provinces in China.

Weakness; fatigue; anorexia; abdominal pain; low fever; dizziness; weight loss; GI symptoms

Causes and Pathogenesis

Hermaphroditic fluke; life cycle: adult/egg/miracidium/sporocyst/redia/cercaria/metacercaria. Adult in intrahepatic bile ducts (branches; rarely pancreatic ducts). Flat translucent brown 10-25 × 3-5 mm; oral/ventral suckers; branched testes. Transmission: raw fish. Metacercariae die at 98°C 1 s / 70°C 5-8 s; resistant to vinegar (2 h)/soy sauce (5 h).

Preventive Measures

(1) Screen/treat; don't feed raw fish/fish offal to cat/dog/pig; manage animal feces. (2) No raw/undercooked fish; separate raw/cooked cutting boards; no grilled/roasted/dried raw fish; no raw nightsoil into fish ponds.
