Gnathostomiasis

Gnathostomiasis: G. spinigerum L3 larva migrans; migratory subcutaneous nodules + eosinophilia; visceral (brain/lung/eye/liver/kidney).

Common Symptoms

Diarrhea; melena; abdominal pain; fatigue; low fever; creeping eruption; nausea

Causes and Pathogenesis

Adult bright red, male 11-25 mm, female 25-54 mm; head bulb with 4-8 rows hooks; gastric tumors in cats/dogs. Eggs 40 × 70 µm, operculated. Water → L1 → Cyclops (1st IH) → L2 (7-10 d) → fish/frog/snake/turtle/eel/loach (2nd IH) → L3 (1 month). Paratenic hosts (chicken/duck/pig/tiger/leopard/wolf). Human ingestion → L3 in gastric wall → liver/muscle/connective tissue; cannot mature; migrates for years (up to 10+). Pathology: eosinophilic parasitic granuloma; toxic/allergic metabolites.

Treatment

Drug therapy, supportive therapy

Preventive Measures

No raw freshwater fish/eel/turtle/frog/chicken/pork. CNS form mortality 2.5-7.7%.
