Nodal Sarcoidosis

Nodal sarcoidosis is noncaseating granulomatous lymphadenopathy in sarcoidosis, presenting with painless bilateral hilar/mediastinal/cervical lymphadenopathy; it is a common early sarcoid finding and may be incidental on chest X-ray.

Common Symptoms

Painless bilateral hilar lymphadenopathy; cervical nodes; fever; fatigue; erythema nodosum

Causes and Pathogenesis

Unknown antigen triggers T-cell/monocyte granulomatous inflammation; noncaseating granulomas in lymph nodes/lung.

Treatment

Observation; corticosteroids if symptomatic/organ-threatening

Preventive Measures

Follow chest X-ray; avoid tobacco; monitor organ involvement.
