Cavernous Transformation of Portal Vein

消化内科

Common symptoms

上消化道出血、腹水、静脉曲张、阻塞性黄疸、反复呕血

The pathology of disease

Cavernous transformation of the portal vein is cavernous collateral vessel formation after portal-vein thrombosis/occlusion, causing portal hypertension, splenomegaly, variceal bleeding, and abdominal pain. Treatment: anticoagulation (if not bleeding); treat underlying cause (cirrhosis/malignancy/thrombophilia); manage portal-hypertension complications (variceal bleeding - band ligation/β-blockers); TIPS or shunt surgery when indicated; monitor liver function; avoid anticoagulation in active variceal bleeding.

Common Symptoms

Portal HTN; variceal bleeding; splenomegaly; abdominal pain; post-PVT

Causes and Pathogenesis

Portal-vein thrombosis causes cavernous collateral recanalization, leading to portal hypertension.

Treatment

Anticoagulation if not bleeding; treat cause; manage portal HTN; TIPS/shunt when indicated

Preventive Measures

Treat PVT early; screen thrombophilia/malignancy.
