Cholelithiasis

Cholelithiasis is formation of gallstones in gallbladder (cholesterol/pigment), usually asymptomatic; complications: biliary colic, acute cholecystitis, choledocholithiasis, cholangitis, gallstone pancreatitis. Treatment: asymptomatic: observation; biliary colic: analgesics (NSAIDs/opioids), elective laparoscopic cholecystectomy; acute cholecystitis: NPO/IV fluids/antibiotics, early cholecystectomy; choledocholithiasis/cholangitis: ERCP + sphincterotomy/stent, then cholecystectomy; low-fat diet; monitor recurrence; follow-up.

RUQ pain/biliary colic; nausea; by cholesterol/pigment stones

Causes and Pathogenesis

Cholesterol supersaturation/biliary stasis/nucleation forms stones.

Treatment

Observation if asymptomatic; analgesics/antibiotics; cholecystectomy; ERCP for CBD

Preventive Measures

Low-fat diet; weight control; avoid rapid weight loss.
