Hypersplenism

消化内科

Common symptoms

出血倾向、肝脾肿大、粒细胞减少、脾肾阳虚、血小板减少、头昏、鼻子黄色或棕色、心悸、硬化

The pathology of disease

Hypersplenism is a syndrome consisting of splenomegaly plus one or more cytopenias, with marrow hyperplasia appropriate to the cytopenia; symptoms are relieved by splenectomy. It is secondary to portal hypertension (cirrhosis), hematologic disease (lymphoma, thalassemia), infection (malaria, visceral leishmaniasis), and storage diseases.

Common Symptoms

Splenomegaly; anemia/leukopenia/thrombocytopenia; fatigue; bleeding; left-upper-quadrant discomfort

Causes and Pathogenesis

Enlarged spleen sequesters and destroys blood cells; increased splenic plasma volume and filtering function cause pancytopenia; marrow compensates by hyperplasia.

Treatment

Treat underlying cause; partial splenic embolization; splenectomy for symptomatic cases

Preventive Measures

Treat underlying liver/hematologic/infectious disease; monitor blood counts.

Department to Visit

It is recommended to visit the Hematology department; an appointment can be booked on this platform before the visit.
