Acute transverse myelitis is an acute inflammatory disorder causing bilateral motor, sensory, and autonomic dysfunction below a spinal-cord level, over hours to days. Causes: post-infectious/post-vaccination, demyelinating (MS/NMOSD), and systemic inflammatory. Clinical: acute paraparesis/quadriparesis, sensory level, urinary retention. MRI shows cord lesion. Treatment: high-dose IV steroids, IVIG/plasmapheresis, supportive bladder care, and neurorehab; investigate underlying cause.
Acute paraparesis; sensory level; urinary retention; back pain; bilateral motor/sensory deficit below level
Post-infectious/demyelinating inflammation produces transverse spinal-cord dysfunction.
High-dose IV steroids; IVIG/plasmapheresis; bladder care; neurorehab; investigate MS/NMOSD
Treat infection; early neurorehab; treat underlying disease.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
