Neuromyotonia (Isaacs syndrome) is a rare peripheral-nerve hyperexcitability disorder causing continuous muscle fiber activity, clinical muscle cramps, stiffness, myokymia (undulating muscle), delayed relaxation, and hyperhidrosis. It is often autoimmune (voltage-gated potassium-channel antibodies). EMG shows myokymic discharges. Treatment: anticonvulsants (phenytoin/carbamazepine/gabapentin) reduce symptoms; immunotherapy (IVIG/plasma exchange) in autoimmune cases.
Muscle cramps/stiffness; myokymia; delayed relaxation; hyperhidrosis; myokymic discharges on EMG
Autoimmune/genetic peripheral-nerve hyperexcitability causes continuous motor-unit firing and myokymia.

Medication adherence; immunotherapy follow-up.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
