Sympathetic Cervical Spondylosis

Sympathetic cervical spondylosis is vertebral artery/sympathetic chain irritation by cervical spondylosis. Clinical: vertigo, headache, dizziness, tinnitus, palpitations, chest tightness, visual disturbance, insomnia, aggravated by neck position. Diagnosis: cervical X-ray/MRI, MRA, excluding cardiovascular/otologic causes. Treatment: conservative: NSAIDs, vasoactive agents, beta-blockers if palpitations, PT (cervical stabilization), soft collar, avoid sudden neck rotation; surgical: ACDF/decompression if refractory; manage anxiety/vascular risk; follow-up orthopedics/neurology/cardiology/ENT.

Osteophyte/disc irritation of sympathetic chain/vertebral artery.

Treatment

NSAIDs/vasoactive agents; PT; soft collar; ACDF if refractory
