Fungal Meningitis

神经内科

Common symptoms

呕吐、颈部强硬、剧烈头痛、抽搐、恶心

The pathology of disease

Fungal meningitis is meningeal inflammation, often concurrent with parenchymal infection; deep mycosis. Rising due to antibiotics/steroids/immunosuppressants/transplants/AIDS/pet-keeping. Pathogens: Cryptococcus neoformans/Coccidioides/Blastomyces/Paracoccidioides/Sporothrix/Histoplasma (pathogenic) and Candida/Aspergillus/Zygomycetes/Trichosporon (opportunistic).

Common Symptoms

Vomiting; neck stiffness; severe headache; convulsion; nausea

Causes and Pathogenesis

Rising trends: (1) more long-term broad-spectrum antibiotics/immunosuppressants/steroids/anticancer drugs; (2) longer survival of leukemia/malignancy/diabetes/uremia/immunodeficiency/autoimmune disease; (3) higher clinician suspicion; (4) improved fungal culture diagnostics.

Treatment

Drug therapy, supportive therapy

Preventive Measures

Pigeons = main reservoir; respiratory inhalation of Cryptococcus; boost immunity; avoid long-term broad-spectrum antibiotics/immunosuppressants; treat TB/diabetes; early combined treatment reduces mortality.
