Renovascular hypertension is secondary hypertension due to renal-artery stenosis (atherosclerosis in older men, fibromuscular dysplasia in young women), causing renin-mediated high BP. Clinical: abrupt/accelerating hypertension, abdominal bruit, recurrent flash pulmonary edema, deterioration with ACEi. Treatment: ACE inhibitor/ARB (monitor renal function); renal-artery angioplasty/stenting for atherosclerotic ostial disease or refractory/flash edema; angioplasty alone for FMD; surgical revascularization as needed.
Abrupt/severe hypertension; abdominal bruit; recurrent flash pulmonary edema; age >55 or young female (FMD)
Renal-artery stenosis activates renin-angiotensin-aldosterone, causing hypertension.
ACEi/ARB; renal angioplasty/stenting for ostial atherosclerosis/refractory; angioplasty for FMD
Screen renovascular disease in abrupt/refractory HTN.
It is recommended to visit the Nephrology/Cardiology department; an appointment can be booked on this platform before the visit.
