Mesangioproliferative glomerulonephritis (MsPGN) is a morphologic diagnosis by light microscopy, a group of glomerular diseases with diffuse mesangial cell proliferation and varying matrix increase. It favors older children and adolescents (children 3-15, mean 8; adults mean 25), slightly more males. It is among the few proliferative nephritides causing nephrotic syndrome. Presentation is similar across types; proteinuria and hematuria almost always coexist; proteinuria is non-selective, hematuria often persistent microscopic; 10%-20% have episodic macroscopic hematuria after respiratory infection.
Lumbar pain with renal percussion tenderness; hematuria; chronic renal insufficiency; hematuria with proteinuria
Secondary membranoproliferative GN in mixed cryoglobulinemia has 3 subtypes. Cryoglobulinemia is a monoclonal globulin, usually myeloma protein; type II is monoclonal IgM binding IgG (rheumatoid factor), type III polyclonal. Types II and III damage the kidney, with mesangial proliferation, monocyte infiltration, tram-track basement membrane, and about 1/3 with small-artery vasculitis and microthrombi. C3 nephritic factor (C3NeF), an autoantibody to C3bBb convertase, causes persistent alternative complement activation and low complement. MPGN often recurs after transplant; type II often has HLA-B7.
Medication, supportive treatment
Avoid fatigue, eat reasonably, exercise, and build immunity; treat the primary disease and complications promptly; once infection occurs use sensitive non-nephrotoxic antibiotics and remove foci to prevent progressive renal insufficiency.
It is recommended to visit the nephrology department; before the visit you may first book an appointment and connect with experts through this platform.
