Analgesic nephropathy

肾内科

Common symptoms

面部皮肤皱纹增多、血尿、夜尿增多、食欲减退、胸闷、氮质血症、头晕、多尿、心悸

The pathology of disease

Analgesic nephropathy (AN) is chronic tubulointerstitial damage and/or renal papillary necrosis caused by long-term analgesic abuse when cumulative intake exceeds 1-2 kg, also called analgesic-induced chronic tubulointerstitial nephritis.

Common Symptoms

Increased facial wrinkles; hematuria; nocturia; anorexia; chest tightness; azotemia; dizziness; polyuria; palpitations

Causes

Long-term analgesic use for rheumatic fever, rheumatoid arthritis, migraine or dysmenorrhea leads to dependence; after years to decades with cumulative intake over 1-2 kg the disease develops, and over 6 kg damages the kidney in 50%-80%. Mechanism: components such as acetaminophen accumulate in the medulla and, via the cytochrome P-450 system, generate excess reactive oxygen species while inhibiting prostaglandin synthesis.

Treatment

Stop analgesics, supportive treatment

Prevention

Avoid long-term or excessive analgesic and NSAID use; monitor renal function.
