Mycobacterium kansasii Infection

血液科

Common symptoms

肉芽肿、结节

The pathology of disease

M. kansasii infection was first discovered in Kansas City, USA in 1953 and has since been found worldwide with rising incidence. Water may be its reservoir.

Common Symptoms

Granuloma; nodule

Causes and Pathogenesis

(1) Etiology: M. kansasii is a slow-growing photochromogen growing best at 37°C, forming colonies in 1-3 weeks; no growth at 45°C; S colonies (occasionally R) on egg medium, emulsifiable; no pigment in the dark, but develops red carotenoid pigment after light exposure (its hallmark). Smears show crossed rods with metachromatic granules; nitrate reduction and Tween hydrolysis positive; niacin, catalase, and neutral red negative; inoculation sensitizes mice; intraperitoneal inoculation causes self-limiting granulomas of liver, spleen, and nodes. (2) Pathogenesis: NTM vary in pathogenic potential; as a group they are less virulent than M. tuberculosis and can colonize surfaces without disease; tissue invasion generally occurs in hosts with local/systemic defense defects. M. kansasii invading broken skin causes localized lesions; it mainly causes pulmonary infection.

Treatment

Drug therapy, supportive therapy

Preventive Measures

Standardize medical water, sterile fluids, and liquid chemical disinfectants; ensure correct use, concentration, and contact time; monitor disinfectant concentration. Follow aseptic technique to prevent secondary contamination; label opened infusion solutions; do not share; discard sterile fluids after 24 hours.
