Hodgkin Lymphoma

血液科

Common symptoms

疱疹、淋巴结肿大、驻站医、淋巴液流出、食欲减退、腹痛、血小板减少、免疫性溶血

The pathology of disease

Hodgkin lymphoma (formerly Hodgkin disease/sarcoma) is a malignancy of lymphocytes; named after Thomas Hodgkin who first described it in 1832. Starts in one node group, then spreads to other nodes/extranodal organs/tissues. Histologic hallmark: malignant Reed-Sternberg cells. Chemo/radiation/bone marrow transplant now make it curable.

Common Symptoms

Herpes; lymph-node enlargement; lymphatic discharge; anorexia; abdominal pain; thrombocytopenia; autoimmune hemolysis

Causes and Pathogenesis

Etiology unknown; EBV most studied (EBV genome in RS cells of ~50%; discovered by Epstein & Barr 1964 in Burkitt lymphoma cell line). Monozygotic twin siblings have 99x risk. Immunodeficiency/autoimmune disease increases risk.

Treatment

Drug, supportive therapy

Preventive Measures

Avoid causative factors. (1) Prevent viral infection (EBV/HTLV-1/HIV); prevent colds. (2) Avoid radiation/toxins (benzene, vinyl chloride, rubber, arsenic, gasoline, solvents). (3) Treat immunodeficiency/post-transplant/post-chemo states. (4) Long-term survivors: annual chest/breast exams. (5) Optimism + exercise. (6) High-risk: early diagnosis/treatment; comprehensive nutrition/TPN/blood products; watch TB/fungal/HCV/CMV.
