Allergic purpura, i.e. Henoch-Schonlein purpura (HSP), is a common hypersensitivity small-vessel vasculitis in children. It presents with palpable purpura (lower limbs/buttocks), arthralgia, abdominal pain/hematochezia and renal involvement; it often follows an upper-respiratory infection. Pathologically it is IgA-dominant leukocytoclastic vasculitis of small vessels.
Symmetric palpable purpura; arthralgia; abdominal pain; melena/hematochezia; hematuria/proteinuria
Triggered by infection/drugs/food allergy causing IgA immune-complex deposition in small vessels, causing leukocytoclastic vasculitis of skin, joints, GI tract and kidney.
Self-limited; supportive care; corticosteroids for severe GI/joint symptoms; manage nephritis
Avoid known allergens; treat respiratory infection; monitor renal involvement.
It is recommended to visit the Hematology department; an appointment can be booked on this platform before the visit.
