Neutropenia is defined as absolute neutrophil count below 1.5x10^9/L; agranulocytosis when below 0.5x10^9/L. Causes: drugs (antibiotics, antithyroid, chemotherapy), infection (virions, severe sepsis), marrow disease (aplastic anemia, leukemia, myelodysplasia), autoimmune disease, hypersplenism, congenital neutropenia. Severe neutropenia carries high risk of bacterial/fungal infection (fever, mouth ulcers, sepsis).
Fever; sore mouth/throat; mouth ulcers; infection; fatigue; sepsis
Causes: (1) decreased marrow production; (2) peripheral destruction (immune, drugs); (3) sequestration (hypersplenism); (4) distribution shift. Severe neutropenia impairs innate defense.
Remove offending drug; G-CSF; broad-spectrum antibiotics for febrile neutropenia; treat underlying cause; isolation
Rational drug use; monitor CBC on therapy; infection control in severe neutropenia.
It is recommended to visit the Hematology department; an appointment can be booked on this platform before the visit.
