Gouty Arthritis

Gouty arthritis is acute painful monoarthritis (first MTP/knee/ankle) from monosodium urate crystal deposition, with hyperuricemia. Treatment: acute: NSAIDs (indomethacin/naproxen); colchicine (low-dose); intra-articular/oral corticosteroids; ice; rest. Chronic: urate-lowering therapy (allopurinol/febuxostat); low-purine diet; avoid alcohol/fructose; hydration; weight loss; treat comorbidities; monitor serum uric acid (<6 mg/dL); treat tophi; follow-up rheumatology/internal medicine; distinguish from septic arthritis/pseudogout.
