Gout

Gout is disorder of uric acid metabolism with monosodium urate crystal deposition: acute gouty arthritis, tophi, uric acid nephropathy, nephrolithiasis. Treatment: acute: NSAIDs; colchicine; corticosteroids. Chronic urate-lowering: allopurinol; febuxostat; probenecid; pegloticase. Nonpharmacologic: low-purine diet; avoid alcohol/beer/fructose; hydration (2-3 L/day); weight loss; treat HTN/hyperlipidemia/diabetes/CKD; monitor SUA/renal function; follow-up rheumatology/nephrology/urology.

Acute monoarthritis; tophi; renal stones; hyperuricemia

Causes and Pathogenesis

Uric acid overproduction/underexcretion causes MSU crystal deposition and inflammation.

Treatment

Acute: NSAIDs/colchicine/steroids; chronic: allopurinol/febuxostat; diet; hydration

Preventive Measures

Low-purine diet; avoid alcohol/fructose; hydration; weight loss; treat comorbidities.
