Abdominal Cocoon Syndrome

Abdominal cocoon (sclerosing encapsulating peritonitis) is encapsulation of small bowel by fibrous membrane, idiopathic or secondary (peritoneal dialysis/Crohn/tuberculosis), causing intermittent intestinal obstruction. Treatment: surgical exploration; stripping of fibrous membrane; release of bowel; treat underlying cause (TB: anti-tuberculous; Crohn: biologics; stop peritoneal dialysis); nutritional support; monitor for recurrence/adhesions; follow-up imaging.

Intermittent intestinal obstruction; by idiopathic/PD/Crohn/TB

Causes and Pathogenesis

Fibrous membrane encases small bowel causing obstruction.

Treatment

Surgical membrane stripping; treat underlying cause; nutrition

Preventive Measures

Treat TB/Crohn; avoid chronic peritoneal irritation.
