Hemangioma

普外科

Common symptoms

拇、食、中三指...、弥漫性血管角化、肝淤血、血管畸形、吞咽障碍、呼吸道阻塞、朴玉顺

The pathology of disease

Hemangioma is the most common congenital benign tumor in infants; incidence up to 1% in Mongoloids; unique course of rapid proliferation + spontaneous regression. From residual embryonic endothelial cells; active endothelial sprouts invade adjacent tissue → endothelial cords → canalized → connected to residual vessels; self-contained vascular system (solid tumor). 60% occur in oral/maxillofacial region (facial skin/subcutis/oral mucosa: tongue/lip/floor of mouth); rare in jaw/deep tissue. High incidence + disfigurement/dysfunction → heavy economic/psychological burden; important to diagnose/treat.

Common Symptoms

Diffuse angiokeratoma; hepatic congestion; vascular malformation; swallowing disorder; airway obstruction

Causes and Pathogenesis

Gene-segment dysregulation during early vascular differentiation; early embryonic (8-12 months) mechanical injury → hematopoietic stem cells distribute to other embryonic cells → vascular tissue.extensive proliferation vessels. Embryonic vascular development: plexiform/reticular/truncal stages. Capillary hemangioma (plexiform stop); cavernous hemangioma (reticular fusion). Microscopy: well-formed vascular tissue (intima/media/adventitia), 1-10mm lumen, connective-tissue stroma. Types: (1) Capillary hemangioma: well-formed single-layer endothelium, little stroma/blood cells; localized lobulated, well-demarcated, bright red; blanches poorly. (2) Cavernous hemangioma: large blood-filled vessels, soft, purple-blue, compressible; larger/thicker; expands subcutis/deep neck. (3) Mixed (capillary-cavernous): both features; capillary on surface.

Treatment

Oral/topical meds, intralesional injection, laser, surgical excision, radiation/cryotherapy

Preventive Measures

Congenital, embryo-related; theory links to maternal diet/lifestyle.
