Ureteropelvic junction obstruction

Ureteropelvic junction (UPJ) obstruction is a common obstructive cause of hydronephrosis; obstruction of the UPJ prevents pelvic urine from entering the ureter, impairing pelvic emptying and dilating the collecting system. Initially pelvic smooth muscle proliferates and contracts harder to push urine past the distal obstruction; when increasing peristalsis cannot overcome it, renal parenchyma atrophies and function declines.

Causes

Intraluminal causes include UPJ stricture, valves, polyps and high ureteral orifice; stricture is commonest (87.2%), with muscular hypertrophy and fibrous proliferation, the stricture about 2 cm long and 1-2 mm in diameter, often with a high orifice. The commonest cause is aberrant or accessory vessels from the renal artery or aorta supplying the lower pole crossing and compressing the UPJ.

Treatment

Surgery, supportive treatment

Prevention

Detect and treat hydronephrosis early; congenital UPJ obstruction requires surgical correction.
