Testicular torsion

男科

Common symptoms

阴囊坠胀痛、睾丸静脉曲张、剧烈疼痛、睾丸下垂、恶心、厌食

The pathology of disease

Testicular torsion, or spermatic cord torsion, is violent cremaster contraction on the spirally attached cord after strenuous exercise or blunt scrotal trauma, causing torsion and acute impaired testicular circulation. It often occurs with congenital long mesorchium, poor gubernacular development, cryptorchidism, incomplete descent, incomplete epididymal attachment, excessive epididymal/cord mobility or long cord. It is intratunical or extratunical, usually twisting outward-to-inward. Cryptorchidism is absent testis in the scrotum, including incomplete descent, ectopia and agenesis.

Common Symptoms

Sudden severe testicular pain; scrotal swelling; nausea; vomiting

Causes

Congenital long mesorchium, poor gubernaculum, cryptorchidism and incomplete descent predispose to torsion triggered by exercise or trauma.

Treatment

Emergency detorsion and orchiopexy within 6 hours

Prevention

Urgent surgery for sudden scrotal pain to save the testis.
