Acute epididymitis is nonspecific epididymal infection, the commonest scrotal infection. It usually follows ascending infection from posterior urethritis, prostatitis and seminal vesiculitis along the vas; hematogenous spread is rare. Pathogens are mainly E. coli and staph, common in young/middle-aged men; urethral stricture, improper instrumentation and post-surgery catheters cause it, then lymphatic, hematogenous being rarest. Features: scrotal/epididymal pain, severe cases with fever.
Epididymal cyst; abscess
Two main pathogen groups: STD-urethritis pathogens (Chlamydia trachomatis, gonococcus, Ureaplasma) and enteric gram-negative bacilli.
