Hypertensive intracerebral hemorrhage is spontaneous bleeding into the brain parenchyma, most often in the basal ganglia, thalamus, pons, or cerebellum, due to chronic hypertension and lipohyalinosis of small penetrating arteries. It is a stroke emergency. Clinical: sudden headache, vomiting, focal deficits, decreased consciousness. Treatment: ICU; urgent BP reduction (target ~140 mmHg systolic); reverse coagulopathy; neurosurgical evacuation for cerebellar/large hematomas; manage ICP; secondary stroke prevention (control BP).
Sudden headache/vomiting; focal deficits; decreased consciousness; deep location (basal ganglia/thalamus/pons/cerebellum)
Chronic hypertension damages small penetrating arteries, causing rupture and deep parenchymal hemorrhage.
ICU; urgent BP reduction; reverse coagulopathy; surgical evacuation for cerebellar/large; manage ICP
Strict BP control; secondary stroke prevention.
It is recommended to visit the Neurology/Neurosurgery-Emergency department; an appointment can be booked on this platform before the visit.
