Cerebellopontine-angle meningioma arises from posterior petrous bone/tentorial invasion; excludes clival. Many asymptomatic/incidental; rare multiple/familial. 50% parasagittal; also convexity/falcine; then sphenoid ridge/tuberculum sellae/olfactory groove/CPA/tentorium; rare intraventricular/epidural.
Hoarseness; ataxia; choking; severe pain; neuralgia; tinnitus; CPA syndrome; raised ICP; facial numbness; diminished corneal reflex
Mostly endothelial/fibroblastic; from posterior petrous/medial internal auditory canal; connected to superior/inferior petrosal/sigmoid sinuses; spherical nodular (rare flat). Supply from posterior petrous dura + AICA/PICA. Groups: pre-/post-internal-auditory-canal.
Surgery, rehabilitation
Surgery = only effective method. Unilateral suboccipital craniotomy + CSF release avoids cerebellar retraction; exposes lower CNs/tentorial incisure/midbrain/upper basilar/CN III. Combined temporal + suboccipital approach for better exposure. Trans-tentorial approach = wide field, but risks brain/Labbe-vein injury → postoperative epilepsy/hemiparesis.
It is recommended to visit the Neurosurgery department; an appointment can be booked on this platform before the visit.
