Osteomyelitis of the calcaneus

骨外科

Common symptoms

脚外侧疼、足跟淤血、脚后跟长骨刺、足跟痛

The pathology of disease

The calcaneus is the largest tarsal bone, mainly cancellous with only a thin cortical shell. When standing, the calcaneus is the lowest point; gravity slows venous return, favoring bacterial retention and growth, especially in the central triangular area of the calcaneal body where trabeculae are sparse and marrow is rich, a structural weak point, making it a common site of hematogenous osteomyelitis.

Common Symptoms

Lateral foot pain; heel bruising; heel spur; heel pain

Causes

When standing, the calcaneus is the lowest point; gravity slows venous return, favoring bacterial retention and growth. After epiphyseal closure in adults, hematogenous osteomyelitis mostly occurs in vertebrae, ilium or calcaneus rich in red marrow, forming a focus that spreads outward; because the calcaneal periosteum is tightly and firmly attached, large subperiosteal abscesses rarely form before cortical perforation, but once the cortex is breached a sinus easily forms.

Treatment

Medication, supportive treatment

Prevention

This is caused by trauma with no effective preventive measures; safe production and avoiding injury are key.
