Fungal arthritis

骨外科

Common symptoms

伤口感染、无力、胸痛、脓疱、结节、软组织肿胀

The pathology of disease

Fungal arthritis, also called mycotic arthritis, is a musculoskeletal lesion caused by fungal infection; it is rare, but pathogenic and opportunistic fungal infections and new species are increasing, especially in immunocompromised patients. Chronic evolution and delayed diagnosis are common. Fungi are common causes of chronic granulomatous arthritis. Common fungi affecting the bones and joints include Coccidioides immitis, Histoplasma capsulatum, Blastomyces dermatitidis, Sporothrix schenckii, and in immunocompromised patients Candida, Aspergillus, Cryptococcus and Histoplasma.

Common symptoms

Wound infection; weakness; chest pain; pustules; nodules; soft tissue swelling

Causes

Fungi are common causes of chronic granulomatous arthritis; various fungi can cause bone and joint lesions. Hematogenous spread to joints is uncommon; most arise from adjacent bony fungal foci directly extending into joints. Sporothrix may also enter joints directly through skin/subcutaneous tissue. Coccidioides is inhaled into the lungs or enters through minor skin wounds causing primary pulmonary or cutaneous infection.

Treatment

Supportive care, rehabilitation

Prevention

(1) Eliminate or avoid causative factors, improve living environment, maintain good habits, prevent infection, and eat a balanced diet. (2) Exercise to improve resistance; avoid over-fatigue; stop smoking and alcohol. (3) Early detection, diagnosis and treatment; maintain confidence and adhere to therapy. (4) Provide labor protection for susceptible groups.
