持续性疼痛
Neuropathic arthropathy (NA), also called Charcot joint, is a secondary joint disease caused by loss of proprioception and pain sensation, loss of protective reflexes, and repeated injury.
Persistent pain
Both central and peripheral neuropathy can cause secondary bone-joint disease. Common causes include tabes dorsalis, syringomyelia, diabetes, spinal/peripheral nerve injury, leprosy, multiple sclerosis, meningomyelocele, amyloidosis, congenital analgesia, TB, nerve invasion by tumor, alcoholism, familial dysautonomia, familial interstitial hypertrophic polyneuropathy, and Charcot-Marie-Tooth disease. Tabes dorsalis, syringomyelia and diabetes are the most common causes of NA. Due to nerve dysfunction, deep sensation and pain are lost, posture cannot be adjusted, and joints suffer repeated mechanical injury combined with local neuro-vascular trophic disturbance and poor metabolism, collectively causing degenerative destruction of joint and soft tissue. The neuro-vascular trophic mechanism is now favored: sympathetically mediated altered bone/joint blood flow causes persistent hyperemia and bone resorption, combining with painless repeated injury to produce the joint destruction, fragmentation, osteophyte formation, loose bodies, effusion, capsule thickening, subluxation and dislocation.
Supportive care, rehabilitation
(1) Prevention is the best treatment. Diabetic patients should pay special attention to the feet and ankles, promptly treat minor trauma there, and maintain good glycemic control to reduce diabetic neuropathy and thus the incidence of this disease. (2) Strengthen early syphilis treatment to prevent this disease.
It is recommended to visit the orthopedic surgery department; before the visit you may first book an appointment and connect with experts through this platform.
