Subinvolution of uterus

Subinvolution of the uterus is a common postpartum complication. Normally after delivery, myometrial contraction retracts vessels, narrowing or occluding lumens, reducing local blood supply; ischemic myocytes autolyze and shrink, cytoplasm decreases, the uterus shrinks, the placental attachment shrinks with it, and endometrial regeneration repairs it; the uterus usually returns to near non-pregnant state by 5-6 weeks postpartum, called involution. When this involution fails, subinvolution results.

Prolonged bleeding, enlarged uterus

Causes

Retained placental tissue, infection, or multiple gestation impair normal involution.

Treatment

Uterotonics, treat cause, curettage if needed

Prevention

Ensure complete placental expulsion.
