Endometrial stromal sarcoma (ESS) arises from endometrial stromal cells; ~0.2% of uterine malignancies, 10-15% of uterine sarcomas. Two types: low-grade (LGESS) and undifferentiated (UES, monomorphic/pleomorphic). Presents with abnormal uterine bleeding/pelvic-abdominal mass. LGESS slow, good prognosis; UES high-grade, aggressive, poor prognosis.
Vaginal bleeding; abdominal pain; squamous cell carcinoma
Arises from endometrial stromal cells. LGESS grows along dilated lymphatics/vessels. Gross: polypoid/nodular or myometrial-like; soft fish-flesh cut; worm-like intravascular growth is characteristic of LGESS. Micro: LGESS cells resemble proliferative endometrial stroma, uniform oval/spindle, mitoses 5-10/10HPF, permeates vessels, widespread hyalinization, ER/PR positive, diploid. UES: pleomorphic spindle/polygonal cells, giant cells, mitoses >10-30/10HPF, nests/cords/sheets, infiltrates myometrium; high-grade, early recurrence/metastasis.
