Mooren ulcer is a chronic, progressive, painful peripheral corneal ulceration of presumed autoimmune origin, undermining the corneal epithelium and stroma and progressing circumferentially and centrally. It is not associated with systemic disease (unlike marginal keratitis). Clinical: severe pain, redness, and progressive peripheral ulceration with overhanging edge; may perforate. Treatment: topical/systemic corticosteroids and immunosuppression (cyclosporine/MTX); conjunctival excision; tectonic keratoplasty/peritomy for perforation risk.
Severe pain; progressive peripheral ulcer with overhanging edge; redness; risk of perforation
Autoimmune attack on peripheral corneal stroma causes progressive ulceration, not linked to systemic vasculitis.
Topical/systemic steroids + immunosuppression; conjunctival excision; tectonic keratoplasty
Immunosuppression; tectonic support.
It is recommended to visit the Ophthalmology department; an appointment can be booked on this platform before the visit.
