Malignant glaucoma (ciliary-block aqueous misdirection) is a rare postoperative complication with raised IOP and shallow/flat anterior chamber despite a patent peripheral iridotomy, due to anterior rotation of the ciliary body misdirecting aqueous posteriorly. Treatment: atropine cycloplegia, aqueous suppressants, hyperosmotics; laser/pars-plana vitrectomy if refractory.
Postoperative shallow/flat AC with high IOP; patent iridotomy; pain/blurred vision
Anterior ciliary rotation misdirects aqueous into the vitreous, shallowing the chamber and raising IOP.
Atropine/aqueous suppressants/hyperosmotics; laser/vitrectomy if refractory
Early recognition; cycloplegia.
It is recommended to visit the Ophthalmology department; an appointment can be booked on this platform before the visit.
