Primary acute angle-closure glaucoma is an ophthalmic emergency from abrupt angle closure, presenting with painful red eye, blurred vision/halos, a fixed mid-dilated pupil, nausea/vomiting, and very high IOP. It can permanently damage the optic nerve within hours. Treatment: urgent IOP lowering (topical beta-blocker/alpha-agonist/acetazolamide/hyperosmotic, pilocarpine once IOP lowered); then laser peripheral iridotomy once the cornea clears; treat the fellow eye prophylactically.
Painful red eye; halos/blurred vision; fixed mid-dilated pupil; nausea/vomiting; high IOP
Pupillary block in a shallow-angle eye abruptly closes the angle, raising IOP and damaging the optic nerve.
Urgent IOP lowering; laser peripheral iridotomy once clear; treat fellow eye
Screen narrow angles; prophylactic PI.
It is recommended to visit the Ophthalmology department; an appointment can be booked on this platform before the visit.
