Neovascular glaucoma is severe secondary glaucoma due to fibrovascular proliferation on the iris (rubeosis iridis) and anterior-chamber angle, caused by retinal ischemia from diabetic retinopathy, central retinal-vein occlusion, or ocular ischemic syndrome. It presents with painful raised IOP, poor response to medications, and a guarded prognosis. Treatment: treat underlying retinal ischemia (panretinal photocoagulation/intravitreal anti-VEGF); control IOP medically; glaucoma drainage device/trabeculectomy for refractory; enucleation for blind painful eye.
Painful raised IOP; rubeosis iridis; poor medication response; diabetic retinopathy/CRVO/OIS history
Retinal-ischemia VEGF grows fibrovascular membranes on the iris/angle, closing it and raising IOP.
Treat retinal ischemia (PRP/anti-VEGF); control IOP; drainage device/trabeculectomy; enucleation for blind painful eye
Treat diabetic retinopathy early; PRP timely.
It is recommended to visit the Ophthalmology department; an appointment can be booked on this platform before the visit.
