Paralytic strabismus is ocular misalignment due to weakness of an extraocular muscle or its innervation (oculomotor/trochlear/abducens palsy), presenting with diplopia. Causes: microvascular ischemia (diabetes/hypertension), aneurysm, tumor, trauma, myasthenia. Treatment: investigate the cause (especially CN III with pupil involvement - aneurysm); occlusive prism for symptomatic diplopia; strabismus surgery if stable and persistent.
Diplopia; ocular misalignment; limited eye movement; by nerve (III/IV/VI); vascular/aneurysm/myasthenia cause
Palsy of an extraocular muscle nerve (III/IV/VI) or muscle imbalance causes misalignment and diplopia.
Investigate cause (aneurysm/myasthenia); prism/occlusion for diplopia; strabismus surgery if stable
Control vascular risk; investigate CN III pupil-involving palsy.
It is recommended to visit the Ophthalmology/Strabismus department; an appointment can be booked on this platform before the visit.
